To schedule an appointment, please contact our office appropriate locations. 

Please keep in mind that if for any reason you cannot keep your appointment, call at least 24 hours ahead of time to reschedule.To be fair to those whom attend their appointments on time and to ensure you get your complete treatment, please be on time.

New Patients: We try to see all new patients within 24 hours of the first contact. If you have a referral from your doctor please remember to bring it with you so we have a diagnosis for treatment. Otherwise we can make a courtesy call and get a prescription from your doctor.

To ensure quality service, please allow 1 hour for your first appointment so that we can fully evaluate you and do an initial assessment. You can print and fill out the provided forms below and bring them in with you to avoid waiting time.
